The Local Forms are developed in the Western District of Kentucky for use by the Bar and Public. All local forms are incorporated into the Clerk's Office Administrative Manual. When filing one of these documents, substantial compliance with the Local Bankruptcy Form is required, and Word forms will need to be converted to PDF before uploading to ECF. Other documents on the list are provided for informational purposes and to provide guidance when filing a specific document with the Court. The Local Forms are available in PDF and Word formats. An asterisk next to the PDF form (eg. PDF*) indicates that this form is not available in a fillable format. Also, some forms may have been removed as they are no longer in use.
