Western District of Kentucky - Local Forms

The Local Forms are forms developed here in the Western District of Kentucky for use by the Bar and Public. Forms that have a Local Form letter after their title are forms incorporated into the Clerks Office Administrative Manual. When filing one of these documents, substantial compliance with the Local Bankruptcy Form is required. Other documents on the list are provided for informational purposes and to provide guidance when filing a specific document with the Court.
