Official Bankruptcy Forms and Director's Procedural Forms

The following list of forms includes those Official Bankruptcy Forms and Director's Procedural Forms that are most commonly requested from the Clerk's Office by the bar and public. Hard copies of the forms listed below are available in the Customer Service area of the Clerk's Office in the Court's Louisville divisional office.  

Please be advised that the Court does not provide paper copies of the forms for the Voluntary Petition and bankruptcy schedules. For a list of all forms required to file a new bankruptcy and links to electronic copies of those forms (including the petition and schedules), please visit the Filing Information Web Page.  Additionally, all Official Bankruptcy Forms are available on the web site of the U.S. Judiciary at http://www.uscourts.gov/bkforms/bankruptcy_forms.html.   Forms on these pages can be downloaded free of charge. Alternatively, you may purchase the forms at a local office supply store.
